Spay and Neuter Voucher Application

Please be mindful of our program rules and requirements:

• Spay and neuter vouchers are currently only for dogs. For feline vouchers, please reach out to one of the other wonderful organizations on our resources page.

• In order to be eligible for a spay or neuter voucher, you must be a senior citizen (65 years or older), a United States veteran, or a low-income family with a gross income of $35,000 or less. Proof of eligibility is required.

• Vouchers are limited to 2 per household.

• Vouchers do not cover the cost of vaccines, pain medications, cones, or other services or products.

• Pets 6 years and over may be required to have bloodwork prior to surgery at the pet owner's expense.

• Females in heat, pregnant, or recently had a litter (within 16 weeks of giving birth) may incur additional fees that are not covered by the voucher.

• Vouchers automatically expire 30 days after issuance unless otherwise specified.

• All vouchers are to be redeemed at Humane Society of Mohave County Animal Medical Center in Kingman, AZ.

• Cancelled appointments may void the voucher, at the veterinary clinic's discretion.

By submitting this application, you agree to the above. Please allow us about 7 days to receive and review your application. Once reviewed, we will follow up with the next steps to receive your voucher.
